The premium contributions for all life coverages quoted reflect the current rates and benefit structure as of September 1, 2024. Premium may be paid annually, semi-annually, or quarterly. The premium is based upon the member's and spouse's gender, amount of insurance requested, usage of tobacco/nicotine products, health status and attained age on the date the coverage is issued.

The premium at which you enter the policy is projected to remain the same for the initial 10-year or 20-year term and will not change due to your age or a change in your health status. While premiums are projected to remain level for the initial period while insured, New York Life Insurance Company reserves the right to change rates for all insureds only once in a 12-month period. For the Level Term Life coverages, only non-smokers meeting the highest underwriting standards will qualify for "Preferred" rates. Other non-smokers may qualify for higher "Select" or "Standard" rates. (Note: Smokers may only qualify for "Standard" rates.) Upon approval of your application, you will be notified of the rate classification for each approved person.

The ACOG Group Life coverages are underwritten by New York Life Insurance Company, 51 Madison Ave., New York, NY 10010, under Group Policy No. G-30461-0 on Policy Form GMR-FACE/G-30461-0 (10-year Level Term Life) and under Group Policy No. G-30462-0 on Policy Form GMR-FACE/G-30462-0 (20-year Level Term Life).
